If I install new concrete board and new tile for the bathroom, obviously I need toilet flange extension. The questions:

1. Do the flange must be on top of the tile or just on top of the concrete board is good enough?

2. Do I need to cut the tiles perfectly around the flange? Or just a square hole is OK?

3. Do I need to put caulking or anything like that around the new flange extension?

The top of the flange extension needs to be flush with the top of the finished tile or a little higher - not lower.The flange can overlap the concrete board and it can even overlap the tile. Typically the flange isinstalled and then the tile is cut up to it.

No need for perfection, just make sure that the tile will continue underneath the footprint of the toilet. Any gaps between the tile and flange can be filled with grout.

3. Do I need to put caulking or anything like that around the new flange extension?

As above, fill any gaps with grout.
