if soaking them with vinegar, then steam cleaning with detergent and a repeat with rinse water doesn't do it, the way you attack this is cut the carpet, roll it up, and put it in a dumpster.
fresh urine might clean up. the old stuff... never.

Buy a large container of Nature's Miracle or other urine enzyme cleaner (the large containers are cheaper per ounce). It must be used straight and poured onto and follow down the same route the urine went (soaking area again). This stays wet, and the enzymes work on the urine overnight or hopefully longer. Longer wet time is better, as once the NM dries, it stops working. Place plastic over the area to keep it working longer if necessary.
Repeat applications may be necessary until stain and odor are gone, then steam clean with an extraction cleaner.
This does work, but it takes diligence and you must get down to where ever the urine soaked. Including carpet pad and subfloor if odors are to be eliminated.
If you have a large carpeted area where urine is randomly present, replacement of carpet and pad is the best option. But if you have urine in the subfloor or soaked into the concrete, an enzyme cleaner treatment would be necessary for odor reduction, as it will ghost out when humidity is high.

First you must make sure the dog is not continuing to go in the same places. Dog, and cat urine has a lot of salt content.
A mixture of vinegar and water heated to a boil will work, use a soft bristled brush to apply. It's best to do the top and bottom of the carpet then vacuum then rinse and vacuum again. Normally I just cut the padding out and replace it with a piece from a closet.
Use a fan to dry the carpet, if carpet stays wet too long it may start to delaminate.

who are best at removing stains and odor from the carpet.
